Exploring Life & Business with Matt Cash of Lily Clinic LLC: Lymphedema, Vascular Health and Wellness

Today we’d like to introduce you to Matt Cash.

Hi Matt, we’d love for you to start by introducing yourself.
My path to Lily Clinic was not a straight line, and I never originally imagined myself becoming a business owner. I began my professional journey in social work, earning my undergraduate degree from West Chester University in 2003. That experience taught me how deeply a person’s health is affected by access to care, family support, finances, and the ability to navigate an often-overwhelming healthcare system. Wanting to help people in a more hands-on way, I later earned my master’s degree in occupational therapy from Misericordia University in 2013.

Early in my occupational therapy career, I worked with patients experiencing a wide range of physical and functional challenges. In 2017, I completed specialized training in lymphedema therapy and became board-certified by the Lymphology Association of North America. I quickly realized that I had found the population I was meant to serve. Lymphedema is frequently misunderstood, misdiagnosed, and inadequately treated. Many patients live for years with painful swelling, wounds, recurrent infections, limited mobility, and significant changes in their quality of life before they finally reach someone who understands their condition.

As my practice evolved, I became increasingly involved in treating patients with venous disease, chronic wounds, lipedema and complex swelling. I expanded my education by becoming a Certified Wound Therapist, allowing me to better address the close relationship between uncontrolled edema, impaired skin integrity, and chronic wound development. This combination of lymphedema and wound-care expertise became the foundation of my clinical work.

The turning point came when the local hospital where I worked abruptly closed its wound-care center and later eliminated its lymphedema program. I was laid off in July of 2025. The closure did not eliminate the community’s need for care—it simply eliminated one of the few places where patients could receive it. I knew these patients still needed somewhere to go, and I could not walk away from them. That experience pushed me to take an enormous professional and personal risk by creating something of my own. Lily Clinic!

Lily Clinic LLC for Lymphedema, Vascular Health and Wellness cared for our first patient in December 2025. The clinic was founded to provide specialized, compassionate care for people living with lymphedema, lipedema, venous insufficiency, chronic wounds, postsurgical swelling, and other complex edema-related conditions. My goal has always been to create an environment where patients feel heard and understood—particularly those who have spent years being told that nothing could be done.

Since opening the clinic, the mission has grown beyond the walls of the practice. Through social media, patient education, the Lily Pod podcast, and the development of a community-supported worldwide lymphedema clinic directory, I have worked to increase awareness and connect people with appropriate resources. What began as a response to the loss of a local hospital program has become a much broader effort to advocate for a patient population that has remained overlooked for far too long.

I arrived where I am today through a combination of education, unexpected professional loss, determination, and the trust patients placed in me. Lily Clinic exists because I believed this community deserved more than being told that their condition was something they simply had to live with.

We all face challenges, but looking back would you describe it as a relatively smooth road?
It has not been a smooth road at all. Throughout my career, I have repeatedly challenged larger healthcare systems to provide the level of lymphedema treatment patients genuinely need. Too often, decisions were driven by supply costs, productivity expectations, and rigid scheduling rather than clinical necessity. Therapists were expected to accomplish complex treatment in 45-minute appointments when many patients needed at least an hour—and sometimes considerably longer—to receive appropriate manual therapy, compression, wound care, education, and training.

I consistently advocated against cutting corners, particularly when it came to compression supplies and the time required to treat patients safely and effectively. That was not always easy or popular. When the hospital lymphedema program disappeared, the patients and their conditions did not. They were simply left with even fewer options.

Opening Lily Clinic brought an entirely different set of challenges. I had extensive clinical experience but no formal business education and very little understanding of what it took to build and operate a private practice. I had to learn everything—from insurance credentialing, billing, payroll, compliance, and purchasing to marketing, staffing, and simply keeping the doors open. I did not feel completely prepared, but I also felt that walking away was not an option.

I fought tooth and nail to get where I am today. I pushed past the people who told me it could not be done, questioned systems that were not serving patients, learned from mistakes, and discovered who my true friends, mentors, and professional allies were. Building this clinic has required persistence, sacrifice, and a willingness to keep moving forward even when I did not have every answer.

Lily Clinic began as a business of one (and one very crucial, supportive and the love of my life 2nd partner Amanda, my loving wife). Today, we reached an important milestone by adding a second therapist. We also have a much-needed part-time front-desk team member who has become the backbone of the clinic and allows the therapists to concentrate on patient care. As we continue to grow and add personnel, our purpose remains unchanged: to provide people with what they truly need—thorough, compassionate, and clinically appropriate lymphedema therapy without cutting corners.

Great, so let’s talk business. Can you tell our readers more about what you do and what you think sets you apart from others?
Lily Clinic LLC for Lymphedema, Vascular Health and Wellness is a specialized occupational therapy clinic serving individuals with lymphedema, lipedema, lipo-lymphedema, venous insufficiency, phlebo-lymphedema, chronic wounds, postsurgical swelling, and other complex edema-related conditions. Many of our patients have spent years searching for answers or have been told that little could be done for them. We provide a place where their symptoms are recognized, their concerns are taken seriously, and their treatment is tailored to their individual needs.

We specialize in complete decongestive therapy, which may include manual lymphatic drainage, compression bandaging, skin and wound care, therapeutic exercise, patient and caregiver education, and development of a long-term home-management program. We also evaluate and measure patients for compression garments, provide training in their use, coordinate pneumatic compression devices, and collaborate with physicians, wound-care providers, vascular specialists, durable medical equipment companies, and caregivers.

One of the clinic’s greatest strengths is the ability to address the relationship between swelling and wounds. Uncontrolled edema can impair skin integrity, delay wound healing, contribute to lymphorrhea, and increase the risk of infection. Because I am both a LANA-certified lymphedema therapist and a Certified Wound Therapist, I can approach these problems together rather than treating the swelling and wounds as completely separate conditions.

What sets Lily Clinic apart is that we do not believe in a one-size-fits-all approach or in cutting treatment short simply to meet a productivity target. Some patients need more time, additional education, creative problem-solving, or collaboration with several medical providers. We are willing to do that work. Our patients are not treated like numbers on a schedule. They are active participants in their treatment, and we take the time to help them understand their condition and learn how to manage it successfully at home.

We are also known for taking on complicated cases that may require unconventional solutions. That can mean adapting a compression system to a patient’s abilities, involving family members in treatment, finding ways to accommodate unusual limb shapes, addressing wounds and severe skin changes, or working directly with manufacturers to explore customized products when standard options are inadequate. We may not always have an immediate solution, but we are willing to keep looking for one.

Beyond clinical care, education and raising awareness have become important parts of the Lily Clinic brand. Through our social-media community, patient education videos, the Lily Pod podcast, and a growing community-supported worldwide lymphedema clinic directory, we work to make reliable information and treatment resources easier to find. We use approachable language, humor, and real patient experiences—with permission—to help people understand conditions that are too often ignored or misunderstood.

I am most proud that Lily Clinic has become a trusted place for people who previously felt overlooked. The brand has grown through patient relationships, community support, and a genuine commitment to doing the right thing clinically. We recently expanded from one treating therapist to two and are continuing to build a team capable of reaching more patients without sacrificing the quality or personal attention on which the clinic was founded.

What I want readers to know is that lymphedema , lipedema and chronic venous insufficiency are not merely cosmetic concerns. They can affect mobility, independence, skin integrity, wound healing, infection risk, employment, and overall quality of life. Appropriate treatment can make a meaningful difference. Lily Clinic exists to provide that treatment, advocate for this underserved population, and remind patients that they deserve to be heard, educated, and cared for properly.

How can people work with you, collaborate with you or support you?
There are many ways people can work with, collaborate with, or support Lily Clinic. Patients and families can visit our website at www.lilyclinicllc.com to learn more about evaluations, treatment, compression options, wound-related services, international clinic directory and education for lymphedema, lipedema, venous insufficiency, postsurgical swelling, and other edema-related conditions.

We welcome referrals and collaboration from physicians, nurse practitioners, physician assistants, wound-care providers, vascular specialists, oncologists, surgeons, occupational and physical therapists, home-health agencies, case managers, and durable medical equipment providers. These conditions are rarely managed successfully by one professional working alone. Communication between the patient, therapist, medical providers, caregivers, and equipment companies is often essential to achieving the best possible outcome.

I am also interested in collaborating with healthcare organizations, professional associations, universities, advocacy groups, manufacturers, and community programs that want to improve lymphedema education and access to care. I welcome opportunities to provide professional education, speak to students or healthcare providers, participate in podcasts and interviews, and contribute to conversations about lymphedema, lipedema, wound care, compression, and the barriers patients encounter within the healthcare system.

Clinics and qualified lymphedema professionals worldwide can support our community-based directory by submitting accurate and verifiable practice information. The directory was created to help people locate care when they do not know where to begin. It is an evolving public resource, and its value depends on professionals and patients helping us identify legitimate treatment locations. It can be found at www.lilyclinicllc.com/directory.

People can also support Lily Clinic by following and sharing our educational content on YouTube and Facebook, listening to the Lily Pod podcast, recommending our resources to someone who may need help, and increasing awareness that chronic swelling is a medical condition deserving appropriate treatment. Sharing one educational post may help someone recognize their symptoms or discover treatment after years of being dismissed.

For patients who have had a positive experience with the clinic, honest reviews and personal recommendations are incredibly meaningful to a small practice. Much of our growth has occurred because patients and healthcare providers trusted us enough to tell others about the care they received.

Support does not always have to be financial. Collaboration, referrals, shared knowledge, professional mentorship, community partnerships, and simply helping us amplify the message all strengthen our mission. The best way to support Lily Clinic is to help us ensure that people living with lymphedema, lipedema and related conditions are seen, believed, and connected with appropriate care.
