Today we’d like to introduce you to Erin Franklin Spengler.
Hi Erin, we’d love for you to start by introducing yourself.
My story began with a lifelong interest in movement, health, and the human body. I studied exercise science in college and competed at the Division I level in cross country and track, which naturally drew me toward a career in physical therapy. I earned my doctorate in physical therapy from Duke University and moved to Northern Virginia after graduation to pursue an orthopedic residency program, culminating in board certification as an orthopedic clinical specialist.
Over the years, I started noticing a shift in the profession and work became more focused on productivity than on patients. I knew I didn’t want to practice in that kind of environment, and that realization inspired me to start my own clinic, where I could provide truly personalized, one-on-one care. I opened Franklin Physiotherapy in August 2025 as a cash-based, concierge, mobile practice. I still offer home visits, but I now have a dedicated clinic space in the heart of Old Town Warrenton. My goal was to create a space and experience that felt different from the traditional physical therapy model. Patients are never handed off to an aide, split between other patients, or limited by insurance authorizations and visit caps. The clinic also includes functional strength training equipment from companies like Rogue and REP, allowing me to load patients in ways that reflect the real demands of everyday life and sport.
In recent years, I’ve received advanced training in women’s pelvic health, and combined with my orthopedic and athletic background, I’ve established a niche in serving active women in Warrenton. I want to be a trusted resource for this underserved population and help women understand that they do not have to live with pain, pregnancy or postpartum challenges, incontinence, prolapse, or other issues that keep them from doing the activities they love.
I’m sure it wasn’t obstacle-free, but would you say the journey has been fairly smooth so far?
It has not been a smooth road. Building a business from scratch has been challenging in ways I didn’t fully anticipate. My background has always been in patient facing roles, so learning how to navigate the business, legal, and marketing sides of running a practice while continuing to grow clinically, has been a steep learning curve. At the same time, it has also been incredibly rewarding to build something that reflects the kind of care I believe patients deserve.
As you know, we’re big fans of Franklin Physiotherapy. For our readers who might not be as familiar what can you tell them about the brand?
Franklin Physiotherapy is a physical therapy clinic in Warrenton specializing in orthopedics and women’s pelvic health. I provide holistic, one-on-one care that supports active adults, with a particular focus on active women, through every stage of life, from injury and postpartum recovery to pelvic health concerns. My motto is “Feel better, move better, stay strong,” and my mission is to help patients restore movement, relieve pain, and build lifelong strength through expert, compassionate care.
I think what sets me apart from others in the area, aside from one-on-one care, is the flexibility of the services I provide. I meet the patient where they are, whether that’s at home, in clinic, or virtually. This is a level of accessibility most clinics don’t offer. My clinic also offers a gym-based setting so patients can build real, meaningful strength. Additionally, I bring a deep, personal level of understanding to injury, rehab, and recovery from my background as a high level athlete, which is a unique combination and one that shapes how I provide care to my patients. Ultimately, I want people in this community to know that expert, personalized care is available to them without the limitations and busyness of an insurance-based clinic. Whether someone is recovering from injury, returning to sport, or navigating pain that has been dismissed or overlooked, especially the pelvic health issues women are told to live with, I want them to know they have someplace to go.
Who else deserves credit in your story?
I owe a lot of credit to my husband, who has helped tremendously with the behind-the-scenes parts of the business, including administrative tasks and marketing. I’ve also been fortunate to receive guidance from my former boss, who helped me think through larger business decisions, and from a former student, now a successful clinic owner, who helped me better navigate the cash-based physical therapy world. My family and friends have also been incredible supporters and cheerleaders throughout this process.
